COSAC 2009 : Online Registration Form

Please fill in the details below to register for COSAC 2009. You may supply details for up to two people on this form. Following your registration we will send a letter to confirm your details and total amount due.

REGISTRATION DETAILS:
FULL REGISTRATION:

(includes all morning & afternoon teas, lunches & dinners)
 
Note:  When registering for two people you can select one rate that applies to both of you, or two different rates.

ONSITE ACCOMMODATION:

(Bed and breakfast for two days)

DAY REGISTRATION:

(Day registration booked after 21 August cannot be guaranteed)

PAYMENT DETAILS:

Note: Cheque payable to ISCAST Ltd (Vic). Credit card payments will incur an additional processing fee of $5. For EFT and credit card payments the office will contact you following your registration.
